What is Reconnections?
|Reconnections is a new contract awarded to us, in partnership with Age UK, to combat loneliness in Worcestershire. This is a pilot project funded by major investors in the aim to reduce the costs on the NHS and their services as a result of a person being lonely. For example they may visit the doctors more often for company, they may become recluse and result in physical or mental health issues which then impact on NHS costs and services, if the person was identified as lonely in the first instance and offered support then these health issues may have been avoidable.
AgeUK hold the contract and pay Deaf Direct for each client that we assess and put through a Reconnections plan, the crucial parts of the client assessments are 4 questions which hold a scoring system. The client is asked the 4 questions at the start, during the plan and towards the end. The idea being that the loneliness score goes down as they are reintroduced into the community.
If you are aware of someone that may be lonely please do make a referral to Amanda or Clancy, if possible get the persons permission for us to contact them and the best method of doing so.
