Personal Information Prescription
- What is a Personal Information Prescription?
- If you have questions about a condition you have and would like advice or further information, you can visit a health or social care professional and recieve a Personal Information Prescription (PiP). You will be given a collection of information related to your enquiry. You may take it home, read through it, think it over, and contact the relevant professionals or equipment suppliers.
- If you live in the Oxfordshire area, you may request a PiP from any of the following places:
- Oxfordshire County Council Hearing Impairment Team
- Oxford Radcliffe Hospitals Audiology Department
- Deaf Direct Oxford - The Oxford Deaf and Hard of Hearing Centre
- Oxfordshire County Council Library Services
PiP-Ox is an initiative funded by the Department of Health in partnership with Oxfordshire County Council, Oxford Radcliffe Hospitals NHS Trust Audiology Department and Deaf Direct Oxford.
